1. Important Notice Regarding Protected Health Information

Our website contact form, general inquiry form, standard email, and standard SMS/text messaging are not intended for the submission of protected health information (“PHI”) unless we specifically direct you to use a secure, HIPAA-compliant communication method.

PHI includes, but is not limited to:

Please do not send PHI through general email, website contact forms, social media messaging, or standard text messaging unless you have been instructed to use a secure, HIPAA-compliant communication channel.

A. Information You Provide Directly

We may collect information that you voluntarily provide to us, including:

Clinical and demographic information submitted through FormDR is governed by our Notice of Privacy Practices, applicable HIPAA regulations, and our Business Associate Agreement with FormDR.

Our website is built and hosted on the Webflow platform. Webflow may collect limited technical data, such as IP addresses, necessary to deliver the site securely. Webflow is not a HIPAA Business Associate and does not process PHI on our behalf.

FormDR

Clinical intake forms are hosted by FormDR, a HIPAA-compliant electronic forms platform. FormDR intake forms are not embedded on our website and are provided directly to parents, guardians, or clients after they contact us, request an intake form, or wish to begin services. Radiance Pediatric Therapy has executed a Business Associate Agreement with FormDR. PHI submitted through FormDR is governed by that agreement, our Notice of Privacy Practices, and applicable HIPAA regulations.

Google Tag Manager

We use Google Tag Manager to manage and deploy marketing and analytics tags on our website. Google Tag Manager itself does not collect or store personal data but enables the tools described below.

Google Analytics 4

We use Google Analytics 4 to analyze website traffic and visitor behavior in aggregate. Google Analytics 4 uses cookies and similar technologies. Data collected is used for analytics and website improvement. We do not use Google Analytics 4 in a way that allows Google to identify individual users from our site in connection with their health information.

Microsoft Clarity

We use Microsoft Clarity to understand how visitors interact with our website through heatmaps and session recordings. Clarity may capture mouse movements, clicks, scrolling behavior, and similar usage activity. We configure Clarity to mask form field content so that text entered into forms is not captured.

Google Search Console

We use Google Search Console to monitor and maintain our site’s presence in Google Search results. This tool does not collect personal data from site visitors directly.

Telecommunications and Messaging Providers

We use RingCentral as our telecommunications and messaging service provider to support phone, voicemail, and SMS/text messaging communications, and we have executed a Business Associate Agreement with RingCentral. RingCentral may process limited information necessary to send, receive, route, and manage communications on our behalf, such as phone numbers, message content, delivery status, timestamps, and opt-out records.

RingCentral is not authorized to use your mobile phone number, SMS consent, or text messaging information for its own marketing or promotional purposes.

4. How We Use Your Information
